  • Type: Part-time, non-exempt position requiring 22-30 hrs./wkly

    Job Summary:

    We are seeking a dedicated Case Manager to join our team. This role will take on a coaching role for DFH clients. The ideal candidate will be responsible for the implimentation of Pathways to growth motivational coaching program to provide support and advocacy to clients in need, ensuring they receive the necessary resources and assistance to improve their quality of life.

    Duties:

    • Support clients by enrolling new clients to our food pantry and answer questions about our pantry hours and programs.

    • Assist families who live outside of our service area by connecting them with food pantry resources.

    • Interact with clients during food distribution hours to recruit for Pathways to Growth, and other internal programs/workshops and classes..

    • Schedule regular appointments and followup with Pathways to Growth participants.

    • Connect clients with appropriate services and resources both withing DFH and in the wider community (e.g. housing, healthcare, education, and employment).

    • Monitor and mentor client's progress, document milestones, and adjust care plans as needed to support goal achievement.

    • Maintain accurate case notes and documentation in teh case management software system.

    • Build and maintain relationships with community organizations to identify resources and coordinate beneficial workshops or refferals.

    • Maintian internal resource database with with current, reliable information.

    Qualifications:

    Associate’s-level or higher degree in Social Work or relevant work experience in behavioral health, case management, client intake/referral services, or related field.

    Skills:

    - Experienced in Client intake, assessment and case documentation

    -Profficient in Google Workspace and Microsoft Office

    -Skilled in data tracking, reporting and navigating multiple software systems

    -Experience in refferral coordination and resource navigation

    -Ability to manage caseloads and maintain consistent client follow-up

    -Strong written and verbal communication skills
    - Knowledge of assessment and charting tools

    -Excellent interpersonal and relationship-building skills

    -Highly organized with strong attention to detail
    - Effective time management and ability to meet deadlines

    Expected hours: 22 – 30 per week

    Tuesday, Thursday, Saturday, Every other Wednesday.

    Occasional evening and weekends to support programs and workshops

    Work Location: On site. This position is not remote.
